“I understand the plan, but I need help with a specific next step.”
A focused service for patients or caregivers who need help navigating a specific follow-up task after discharge.

Follow-up navigationHelp with one primary follow-up need per session.
Referral navigationHelp with one primary referral-related need per session.
A supportive session to help patients or caregivers organize medications according to the medication instructions already provided by their prescribing healthcare professionals.
Review of the medication list provided to the patient
Organization of medication timing
Creation of a medication schedule/checklist
Identification of questions or discrepancies that should be addressed with the prescriber or pharmacist
This focused session provides more dedicated time for medication organization than the medication-information support included in the Discharge Clarity Session. It focuses on organizing medication information already provided by healthcare professionals and ends with a written schedule or checklist.
Xzora Solutions does not independently prescribe, discontinue, increase, decrease, or change medications through this navigation service.
Final pricing may vary based on the complexity of the situation. Any additional services will be discussed with you before proceeding.
Each service can be booked directly through our secure Square scheduler, which opens in a new tab. If you're not sure which service fits, start with the free 15-minute call or send us a message and we'll help you choose.
Real-life situations
Which sounds most like your situation?
“My mother just came home from a skilled nursing facility and I'm overwhelmed by everything she was sent home with.”
These are illustrative examples of the kind of work each service involves. They are not actual client situations.
Discharge Clarity Session
$129
What arrives
A discharge packet from a four-day hospital stay. Eleven pages, six of them printed instruction sheets. Seven medications listed, two of which appear to replace pills already in the cabinet at home. A follow-up appointment noted as “2 weeks” with no date and no phone number.
What we do
Read the packet through together, line by line, translating the clinical shorthand into plain language. Build a single medication reference that separates what is new, what continues, and what looks like it may duplicate something already at home — flagged as a question for the prescriber rather than an answer. Identify which follow-up is time-sensitive and which office to call. Write down the questions worth raising at the next appointment.
What you leave with
One written action plan, one medication reference sheet, a next-step checklist in date order, and a written list of questions with the right person named beside each one.
A referral to a specialist was placed at discharge two weeks ago. No appointment exists. Two calls to the office ended in voicemail, and it is unclear whether the referral was ever received.
What we do
Confirm which office holds the referral and what they still need. Establish whether the records arrived. Get an actual date on the calendar, and write down what to bring and what to expect at that first visit.
What you leave with
A confirmed appointment with a date and time, the direct number and contact name for that office, and a written understanding of what to bring.
$59 covers one primary follow-up need like this one. Several tasks at once is usually a better fit for Complete Post-Discharge Support.
A parent home from a skilled nursing facility after a three-week stay. Nine medications on the discharge list, plus a bag of bottles from before the admission. Three referrals, none scheduled. Home therapy mentioned but not arranged. An adult daughter coordinating it from two hours away, working full-time.
What we do
Everything in the Discharge Clarity Session, then keep going: organize the new list alongside what is already in the house and mark every discrepancy as a prescriber or pharmacist question. Work all three referrals to confirmed dates. Confirm the therapy arrangement is actually in motion and identify who owns it. Build a shared transition checklist the family can work from, ordered by what comes first. Then check back in after the first days at home to see what shifted.
What you leave with
One organized written plan for the next steps identified in your session, appointments confirmed rather than assumed, a medication reference with open questions clearly marked, a checklist the whole family can see, and a follow-up check-in already scheduled.
$299 is the base package. More complex situations may need additional services or a custom quote.
That's okay. You don't need to know which service you need before contacting us.
If you're unsure, tell us what happened, what you need help with, and what feels overwhelming. We'll help you determine which Xzora support option may be the best fit.
Xzora Solutions currently offers these services as private-pay support. Payment is made directly to Xzora Solutions. These services are not being presented as insurance-covered benefits.
Navigation and support — not a replacement for your healthcare team.
Xzora Solutions helps patients and families organize healthcare information, understand instructions, navigate follow-up care, and prepare for the next steps. We do not replace your physician, pharmacist, hospital, rehabilitation team, skilled nursing team, emergency services, or other treating healthcare professionals.
Xzora does not independently prescribe or change medications through these navigation services. We help organize medication information and identify questions that should be addressed with the patient's prescribing professional or pharmacist.
You don't have to figure out healthcare alone.
Choose the support that fits your situation — or let us help you decide.
