A supportive session to help patients or caregivers organize medications according to the medication instructions already provided by their prescribing healthcare professionals.

This focused session provides more dedicated time for medication organization than the medication-information support included in the Discharge Clarity Session. It focuses on organizing medication information already provided by healthcare professionals and ends with a written schedule or checklist.
